I have not been able to find any mention of another one of these. I did talk to staff and was told it was a drop on a single monster type during a single invasion level and that by the time it happened few people were at the invasion anymore...

I played OSI back 1999-2001 or so and had most every kind of character. Found UOF over the summer and have been building a stable of Pvp and a few support chars since then. Glad to be back on a populated shard with old school rules (more or less).
