Setting up hotkeys is all what type of character you have and what suits your hands... What is comfortable.

Cluster your hotkeys so that the most used are in the most convenient spots for you. I like to keep my defensive/healing macros together above my offensive ones.
I have my potions and a few utility macros on my number keys.
Qwerty keys are healing and buff spells
asdfgh are all offensive spells
zxcvbb offensive spells/debuffs
Keys left over on the right are more utility spells/actions
Spacebar usually commit to my most used or important action.
Thats just my setup not sure if it's the most efficient but works for me.

Thank you vm! but what if you want to talk to someone? I'll keep casting spells when I type.. and I can't communicate with party members in stealth.

Disable hotkeys hotkey, will revert back to the letters and nunbers

my layout is very simlar to @Czech Razor i like to keep most of my spells on my left hand but i have a select few on my 6 mouse buttons mostly heals/pots and my breaker spell (mana drain). with target self on mouse wheel down and target last on mouse wheel press.
